What is a Behavioral Health Home?

  • A Behavioral Health Home is NOT a home. It is a service that provides case management to children/ adolescents or adults with a mental health diagnosis who may need additional support or access to resources.

Why Case Management?

  • Need for increased communication between your providers of care

  • You or your child/ adolescent has experienced an increased need for psychiatric inpatient services

  • You or your child/ adolescent has behavioral and medical needs that require additional support than is currently being provided

  • You or your child/ adolescent could benefit from referrals to community resources

  • You or your child/ adolescent have barriers to attending appointments

  • You or your child have any of the following:

  1. Transportation issues

  2. Financial difficulties

  3. Educational difficulties

  4. Counseling services

  5. Medication Management

  6. Food insecurity

  7. Specialty referrals needed such as; psychological evaluations, occupational or speech therapy, weight management services

Behavioral Case Management includes the following services:

  • Comprehensive case management for mental health & medical diagnoses

  • Individual and family support

  • Referrals to community and social support services

  • Coordination of care and access to preventive services

  • Educational& vocational resources and assistance

  • Assistance in transitions from inpatient services to other community services

  • Safety and/or behavior planning

  • Connection to transportation resources

  • Wellness coaching and promotion

  • Aid with food uncertainty

Eligibility for CCS Behavioral Case Management:

Case Management Team:

  • Care Coordinator/ Case Manager:

A provider that will develop a plan with the family to address barriers to treatment. They assist in finding resources within the community to support the individual or family.

 

  • Peer Support Specialist:

A peer that self-identifies as having received or is receiving services related to the diagnosis of a mental illness. This role will collaborate, coach, and challenge individuals in viewing life’s challenges as an opportunity for growth and change; within each individual’s recovery process.

 

  • Family Support Specialist:

A peer that self-identifies as having a child/ adolescent that received or is receiving services related to the diagnosis of a mental illness. This role will collaborate, coach, and challenge individuals and their families in viewing life’s challenges as an opportunity for growth and change; within each individual’s recovery process.

 

  • Nurse Care Manager:

A provider that offers health education to you and your family on medical diagnoses. They offer support on managing illness and preventing hospitalization.

 

  • Administrative Team:

To ensure quality care is provided to you and your family, our Administrative Team is available to our Team Members for support and to monitor service delivery:
